If your child is struggling with sensory processing, emotional regulation, motor skills, feeding, toileting, or sleep, we’re here to help.

At Sensory Pathways 4 Kids, we start with a phone consultation to learn about your child’s needs, answer your questions, and determine whether occupational therapy may be beneficial. If appropriate, we’ll help you schedule a comprehensive evaluation to develop an individualized plan of care.

How to Get Started

  1. Step 1: Complete Our Intake Form

    Download our Patient Intake Form and fill it out as completely as possible. This information helps us understand your child’s needs and prepare for your consultation.

  2. Step 2: Submit Your Form

    Email your completed form to info@sp4kids.com or fax it to 303-756-6059.

  3. Step 3: We’ll Check Your Insurance Benefits

    Once we receive your form, our team will review your insurance benefits and contact you within 24 hours to arrange your phone consultation.

  4. Step 4: Schedule Your Phone Consultation

    During the consultation, a therapist will discuss your concerns, answer your questions, and help determine the appropriate next steps. If a comprehensive evaluation is recommended, we’ll discuss scheduling and what to expect.
